Job Title: Adult Social Worker – Neighbourhood Team – Lambeth
Location: Lambeth (Hybrid)
Pay Rate: £32 per hour
About the Role:
Lambeth Council is seeking an adult social worker to join their Neighbourhood Team. The role involves providing support and assistance to adults in the community, ensuring their well-being and access to necessary services.
What will your responsibilities be?
In this role, you will support the home care mobilization project to transfer service users to their neighbourhood providers. You will review the care needs for adults and speak with service users and their families regarding the change in their care provider, reassuring them throughout the process. Your responsibilities will include ensuring that the transition is smooth and that the service users receive the necessary support and care. This role requires excellent communication skills, empathy, and the ability to manage change effectively to improve the well-being of the service users.

Essential Requirements:
- Social Work England registration
- Eligibility to work in the UK
- Full UK driving license
- Post-qualified experience
- Degree level or equivalent in Social Work
