Job Title: Social Worker – Hospital Discharge
Location: Wakefield
Pay Rate: £35
Liquid Personnel is currently recruiting for an Adult Social Worker to join a Hospital Discharge Team based in Wakefield.
What will your responsibilities be?
- Manage a varied caseload, including capacity assessments, best interest decisions, and Care Act assessments.
- Support adults and their carers with person-centred, strengths-based social care.
- Facilitate safe hospital discharges at Pinderfields or Dewsbury District Hospitals.
- Engage in outcomes-focused, preventative conversations with individuals and their networks.
- Collaborate closely with multi-disciplinary system partners in a fast-paced environment.

Qualifications and Experience:
To be successful in this role you must have registration with the Social Work England (SWE) as a Social Worker, holding a qualification equivalent to the Diploma in Social Work (DipSW).
