We are seeking a highly motivated and enthusiastic individual to effectively deliver and promote our Health Referral Scheme. This is an exciting new role with the opportunity to completely redevelop and manage the scheme across our Freedom Leisure sites in the Ashford Contract.
So that the scheme can be monitored and evaluated effectively you will be working closely with the Healthy Communities Manager & our Fitness Manager to ensure of correct delivery, administration and promotion of the scheme.
Working as a Health Referral Co-ordinator, you will oversee the referral scheme activities and spearhead the development and implementation of various programs, including cancer rehabilitation, dementia care, and menopause movement sessions.
You will be able to build and manage key relationships and cross-sector partnerships across Freedom Leisure, public health and community, voluntary and third sector organisations.

Hours: 20 hours per week, Monday - Sunday (rota basis)
- Level 3 Exercise Referral Qualification
- Proven experience in working alongside patients with a range of health conditions
- Experience in creating, planning and delivering exercise programmes, both one to one and in groups
- An awareness/understanding of local health inequalities
- Able to present and work with a mixture of partners, in the health system and sporting networks
- Well-developed interpersonal skills
- Team-oriented approach, able to work across organisational boundaries and demonstrate interest and be supportive of the work of staff
- Excellent communication skills and adaptability skills
- Excellent time management skills – able to work quickly to assist with the delivery of the club weekly
- Empathetic and social individual who wants to help others improve their lives
- DBS Check
- Mobility to travel across sites
Desirable
- Level 4 Qualification in Cancer Rehab, Pulmonary Rehab or any other relevant
- Safeguarding & Child Protection, First Aid
- At least 12 months experience as a Level 3 Exercise Referral Instructor
