Job Title: Adult Social Worker – Hospital Team – Slough

About the Role:
The primary purpose of the social work role within the Hospital setting within the Adult Social care department is to ensure that vulnerable service users admitted into various acute and secondary hospital setting are discharged safely within the community with a focus on person centred holistic assessments to reflect the goals and outcomes that service users want to achieve.
What will your responsibilities be?
In this role, you will conduct Psychosocial Assessments, evaluate patients’ social, emotional, and environmental needs to develop appropriate care plans. You will collaborate with the healthcare team to create individualized care plans that address patients’ needs and goals. Regularly review and update care plans to ensure they remain effective and relevant. Additionally, you will maintain detailed and accurate case records and documentation. Participation in team meetings, training sessions, and professional development opportunities is also required.

Qualifications and Experience:
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
