Pay Rate: £38 per hour
Job Description:
Liquid Personnel is currently recruiting experienced Qualified Social Workers on behalf of its client to join the Older People’s Mental Health Team and the East Network Team, based in Bridgend, South Wales.
What will your responsibilities be?
In this role, you will,
- Conduct comprehensive assessments of adults in need of care and support
- Develop and review person-centred care plans in line with legislative frameworks
- Work collaboratively with health professionals, families, and community partners
- Manage a capped caseload to ensure quality and timely interventions
- Safeguard vulnerable adults in line with statutory responsibilities
- Maintain accurate and up-to-date case records using electronic systems
- Participate in multi-disciplinary team meetings and contribute to service development

Qualifications & Experience:
To be successful in this role you must have,
- Qualified Social Worker with 2+ years’ experience in Adult Social Care
- Full-time availability with at least 3 days office-based
- Strong commitment to person-centred practice and team collaboration
